After visit/Requisition Forms/Referrals

Lab requisition form only valid for 6 months, cost of duplicate copy $10.00 for lost/expired documents.

1. BLOOD TEST
- please go to Lifelabs or Dynacare– online booking available. Results available online for free with Lifelabs MyCareCompass registration.
- Please fast for 10 hours prior, sipping very small amount of water is allowed. -For Lifelabs, If you do not have access to a printer at home, lab test requisition(s) can be emailed to mailmyREQON@LifeLabs.com.

2. X-RAY/ULTRASOUND
- go to Hitek Imaging for electronic result to come to our office - closest location to our office is Hitek in suite 210 in this building - Appointment is required for Ultrasound - feel free to go to any lab of your choice, but please also make sure to go to the same lab every time for the test as the lab needs to compare your results with the old film, and other lab can send electronic result to our office

3. Referrals-specialist/ hospitals/ test facility/ cardiology test/ CT / MRI
- Please wait 2 weeks - if no call/email received, please call the facility directly. Do not wait for their call if you have not heard from them within that time frame.

4. FIT
-Please check the mailing address is the one you would like to received the FIT package before you leave office. Please call the lab directly if you have not received the package after 2 weeks

5. RESULTS
You can see results from Lifelabs and hospitals for free. Please register for an account at MyCareCompass for Lifelabs results. Please register a Mychart account to see all your hospital results and specialist reports.

Please note- Once you have completed certain tests e.g. blood work, ultrasound, x-ray etc, your doctor will contact you via email/telephone within 5 business days after the result(s) arrived in our office to inform you about the findings. If there is a result that your doctor sees that requires further discussion, you will be notified to make an appointment or your doctor will call you earlier if it is urgent. Please do not book an appointment in advance to review test results unless otherwise told. Some results will arrive at our office in approximately three weeks, such as your immunity status, H-pylori, and hospital test results. If you do not hear back within 4 weeks of your testing, please contact our office and the location where you had the test done to ensure both sides are aware and working on the solution.

6. MEDICAL RECORD FEE -Lab results from Lifelabs can be accessed directly by patient who HAVE registered with the Lifelabs from Lifelabs online portal free of charge. -The actual images referenced in diagnostic imaging report, kindly reach out to the imaging center where the studies were performed

If you want to proceed your request of copies to be facilitated by our office- Please be informed that charges for Reproduction/Transmission of Medical Records as advised by Information and Privacy Commissioner of Ontario $30 (first 20 pages) + $0.25 per page thereafter

7. THIRD PARTY DOCTOR’S NOTE/LETTER/FORM Please wait for an invoice to arrange pre-payment. Rush fee may apply when applicable
